Welcome to our Hospital
To ensure that patients can receive the right care, in the right place, we begin planning for your discharge upon your admission to our wards.
Please tell your nurse or discharge co-ordinator about anything that may affect your discharge straightaway. Once your doctor says you are ready to go home (medically fit), we will aim to get you ready to leave the hospital that same day.
Our team will work with you to ensure you have a smooth and safe discharge. To support us to do this, we ask that you make necessary arrangements in advance with your family or next of kin. This includes transport home if you are not eligible for hospital transport.

Once you are ready to be discharged, your recovery will continue in the most appropriate setting. If you are delayed in leaving you may need to be moved to another ward or site as you do not require an acute hospital bed. In the event that you become medically unwell whilst preparing to leave the hospital, your discharge will be postponed whilst we provide additional care to get you back to being medically fit and able to safely be discharged.
Once everything is ready for you, you will be moved to our Discharge Lounge where you can wait comfortably whilst we finalise your discharge arrangements.
Before leaving the hospital, you will receive a copy of your discharge plan, medications, details of ongoing care, and follow up information.
